It is important that you adhere to the three-month notice period at the end of the contract.

• You can cancel the contract via the Allianz website. Fill out the form, then you can print it out and send it by post.
• In addition to your name and address details, you must have the contract number on hand.
• Send the completed form to: Allianz Private Krankenversicherungs-AG, 10870 Berlin
• Alternatively, you can write the letter yourself. Your data including contract number and date must also be included here.
