AOK: Health insurance card lost - you can do that
If you have lost your AOK health insurance card, you can request a new card from your AOK. In this practical tip, we explain how this works and what you should consider.
AOK health insurance card lost: How to apply for a new card
To apply for a new card, you must contact your AOK:
- AOK Plus: AOK Plus customers can apply for a new card here. Fill out the form completely and send it off. You will receive your new card by post within a few days.
- Customers of regional AOK offices: As a customer of a regional AOK, the easiest way to apply for a new card is by phone or email. To do this, state your name, address and date of birth, and - if known - your insured number. You can find the contact details for your AOK office on the AOK website. You can find the right website for your place of residence using the buttons at the top right.
